New Appointment Protocols
 
Dear Patients,

Due to COVID-19 we request that you take the following screening on behalf of your household prior to your appointment. If your answer is 'YES' to any questions below, please call the office for explanation prior to entering.
1. Do you/they have a fever, or have you/they felt hot or feverish recently in the last 14 days?
2. Are you/they having shortness of breath or other difficulties breathing?
3. Do you/they have a cough?
4. Any other flu-like symptoms, such as gastrointestinal upset, headache or fatigue?
5. Have you/they experienced any recent loss of taste or smell.
6. Have you/they been in contact with any confirmed COVID-19 positive patients? Patients who are well but who have a sick family member at home with COVID-19 should consider postponing elective treatment.
If you can answer 'NO' to the questions above, please feel free
enter the reception room and have your child check in via the screen located next to the front desk. 

We have installed office air purification systems using HEPA filtration to provide medical grade air quality, as well as chairside equipment to reduce the production of aerosols.
Please brush your teeth prior to coming to the office to help minimize the use of our toothbrushing area.
We kindly request that parents and siblings remain in the car while the patient is treated.  If you have a child that needs adult supervision, please limit to one parent per child.  At this time, we are also requesting no additional family in the treatment area.
We encourage you to wear a mask to your appointment.
Upon completion of a child’s appointment, if we need to speak to a parent, we will escort them back to your car. Please call 904-287-5557 if you have any follow up questions.
